RE-ADMISSION OF OLD STUDENTS
Old students shall be re-admitted provided they have satisfactorily complied all the retention requirements set by the program they would like to re-enroll.
Application for Re-Admission shall be accomplished and Application for Accreditation of Subjects (if applicable).
Coordinate with the respective Program Coordinators for proper advise and approval.
Start the enrollment process after the approval of your application.
WITHDRAWAL OF ENROLLMENT
If a student who is officially enrolled decides to quit due to certain circumstances before the start of classes or before the Midterm Examination, an approved request for withdrawal of enrollment must be submitted to the Office of Campus Registrar.
Request for withdrawal of enrollment at the office of Instruction and process all the signatories required on the form.
After approval, proceed to the Office of the Registrar to submit the approved request and retrieve the submitted enrollment credentials for the specified term (if there is any).
APPLICATION FOR LEAVE OF ABSENCE (LOA)
Students who have plans to quit the entire semester or academic year shall coordinate with the Office of Student Affairs and Services for proper guidance on Leave of Absence (LOA).
Students are allowed to avail of the leave of absence for a maximum of two (2) semesters only.
Students who withdraw from the University without formal leave of absence shall forfeit their registration and retention privileges as well as their benefits under RA 10931.
