How to apply PhilHealth Member Registration form/ PMRF Registration process

• First of all, candidates visit its official website philehealth.gov.ph
• Navigate to the online service section and select the option for online registration
• Fill out the Registration form carefully as your full name, birth date, address and contact details.
• Upload the Required Documents in jpeg or PDf formats.
• Before submitting the Registration form Review the Philhealth registration form.
• Submit your application form.
• Receive your PhilHealth number
• Lastly, print your Philhealth ID.

For Newly hired and Existing Employees without PIN yet

• fill out Two (2) copies of the PMRF forms
• Submit PMRF to the HR department of the employer
• await member data record and philhealth ID Card form employer.

report your PIN to your employer for them to indicate the same in their ER2.

PhileHealth Informal Economy Registration:

1. Visit any of the local health insurance offices or Philehealth Outlets
2. fill out Two (2) copies of the PMRF
3. submit PMRF to the LHIO or Philhealth express
4. await member data record (MDR) and PhilHealth ID card
5. pay the necessary premium contribution using your Philhealth ID number
6. Schedule of payment.
